Rehabilitation Member Application Form
Rehabilitation Member Applications are to be submitted by an insurance company or physiotherapist on behalf of their client.
The cost of the three-month membership is $413.60 (including GST) and includes access to:
- 24/7 gym
- Group fitness classes
- Pools
- Spa, sauna and steam
- Warm water pool
- Locker use
- Casual court usage
On your first visit:
- Visit our friendly staff at the reception desk and let them know you hold a rehabilitation membership.
- Provide the opportunity for the reception staff to take your photo for membership identification purposes.
- Collect your RFID band for access to the facilities under this agreement.
