Thank you for watching the Cockburn ARC 24/7 gym induction video. By doing so you should now be clear on our expectations of you as a member and how we can meet your expectations during unstaffed hours.

Please read and agree to the following terms and conditions.

Terms and conditions

You must read, understand and agree to the following terms and conditions

Your Membership provides you access and use of the facilities during unstaffed hours, as such, you must comply with the 24/7 Additional Terms and Conditions.  Nothing in this document limits your other obligations, including the obligation to comply with all of our Terms and Conditions and the Membership agreement. 

1.1 Your physical condition 
(a) If you have any health conditions, when accessing our facilities during unstaffed hours, we require you to seek guidance and provide written authorisation from your general practitioner or an appropriate allied health professional that you are fit to use our facilities during unstaffed hours. 
(b) You promise that information you disclose to us will be true and accurate and not misleading in any way.  You must   not attend or use the facilities whilst you are suffering from any illness, disease, injury or other condition that could present a risk to the health or safety of other members or yourself. 
(c) Prior to using the facilities you are in good physical condition and you do not know of any reason why you may not be able to exercise safely.  If you feel unsure, we ask that you do  not use the facilities until we have completed your health and fitness profile. 

1.2 Proper use of equipment 
You must watch and understand the 24/7 induction video before you use the facilities during unstaffed hours.  You will take care when using the facilities, which includes the appropriate and safe use of equipment. 

1.3 Rules of good behaviour  
We display conditions of entry that apply to everyone using the facilities in order to promote health and safety and the protection of property. You agree to follow the conditions of entry each time you enter the facility.

1.4 We can refuse you entry 
We can refuse you entry to the facilities or cancel your membership immediately if you behave in a way that in our opinion is seriously risky or inappropriate, such as if you threaten or harass others, deliberately or recklessly  damage equipment or facilities or if you use or distribute illegal or performance enhancing drugs. 
You agree that we will refuse you entry to the facilities during unstaffed hours if your membership fees are outstanding, or if your membership mandatory details affecting your payment agreement require updating. 
You also agree that Cockburn ARC may from time to time close the gym overnight to address maintenance, deep cleaning or reformatting.

1.5 Your RFID Access Band
In the instance that your membership RFID band is lost or stolen you must notify the centre immediately to arrange a replacement. A replacement RFID band is $5. 

1.6 Orientation 
You acknowledge that in order for you to use the facilities during unstaffed hours, you will or have undertaken our current orientation or have watched the 24/7 Induction video, which will include the following items: 
(a) how to access the facilities during unstaffed hours; 
(b) the location of the automatic defibrillator, first aid kit, fire extinguishers, emergency exits and assembly area; and 
(c) the location of the duress pendants and alarms. 
(d) prohibited areas 
(e) that you are 18 years or over 

1.7 Duress pendant or alarm 
You agree to: 
(a) only activate the duress pendant or alarm in the event of an emergency, otherwise you will be liable for associated callout charges incurred by the security and emergency service providers. We recommend wearing the private alarm lanyards when you are by yourself in the centre. 

1.8 Use of the facility and equipment 
When using the facility during unstaffed hours, you: 
(a) acknowledge that you enter and use the facilities at your own risk; 
(b) register your attendance via the entry access to the facility; 
(c) must bring a towel for each workout and wear appropriate fully enclosed footwear at all times; 
(d) must ensure there is a spotter when using heavy weights or doing free weight exercises; 
(e) must not allow another person to use your membership RFID Access Band; and
(f) must not allow or enable another person/member to enter the facility or penalties may apply in accordance with your membership terms and conditions. 


1.9 Reporting obligations 
If there is a health and/or safety concern about the equipment or any other concerns regarding the facilities during unstaffed hours, you must report your concerns to us. 
 
The final step in the process is for you to complete the form below and submit it. By doing so you agree to the terms and conditions as well as confirm you have either watched and understood the induction video and read the Induction information and understood it. Once done, your application for 24 / 7 access to Cockburn ARC’s gym will be processed within 3 business days.
