Aquatic Form

Kids Get Active Aquatics Form

The KGA Aquatic Session requires your child to be able to swim in deep water independently. Staff will be present at all times, both in and out of the water to ensure your child is safe.

Has/does your child participate in swimming lessons?:

Note, if not currently in a level, please type 'NA'. If you're unsure about your child's swim level, see the Parent Handbook under the 'Aquatic Participation' section for a reference.
 
Can your child confidently swim in deep water (2 metres) without assistance?:

Acknowledgement:  

  • If your child is not competent, they will be unable to participate and will partake in a dry activity instead.  
  • A brief swimming assessment will be conducted at the start of the session to determine if  your child is competent.
  • KGA and Aquatics staff reserve the right to remove any children from or change/cancel the session at any time if deemed necessary for safety or operational requirements.
