If you are cancelling more than one child, you will need to submit a separate form for each child.

Your cancellation must be lodged by the close of business on Tuesday prior to the next direct debit date to ensure payment is not taken out.
The next direct debit date is Thursday 25th the of April 2024.
If you cancel after Tuesday the 23rd of April 2024, then your cancellation will come into effect two weeks after the above date.

I acknowledge that my membership will cease during the direct debit week ending the 24th of April.
*(If the Tuesday occurs on a public holiday, all forms must be received by the previous Friday)
