If you wish to forfeit a game, please fill in the below form before 5pm on the day of your fixtured match.
Any forfeits after this time must be made over the phone to reception, otherwise an additional fee will apply. You can contact reception on 08 9411 3344.

Please confirm the following:
Forfeits with more than a day's notice will be required to pay a standard game fee ($75 for Senior Social Sports or $64 for Junior Sports).
Teams forfeiting on the day of their game will be required to pay a standard game fee plus the opposition's game fee ($150 for Senior Social Sports or $128 for Junior Sports).
This fee will be charged automatically to the team's nominated credit card.
